patient is discharged from therapy. This survey is used to measure
the patient’s satisfaction with services received.
PATIENT FOLLOW UP
SURVEY
This survey is requested ninety days (90) after
a patient is discharged from therapy. The survey is used to measure
the patient’s durability to treatment received.

our facility with Information and Outcome Management reporting and
strategic planning. The information received is greatly appreciated
because it helps us to determine how we may better serve your agency.
